Middle School Enrollment


To enroll a student at Bethel Middle School please print and complete the Enrollment Packet and locate the required documents before you call, 937.845.9430, to schedule an appointment. 
A parent or legal guardian must be present to complete the enrollment process.

Documents required to enroll:
  • Birth certificate & Social Security Card

  • Proof of residency - Acceptable Proof of Residency 

    • Examples: utility bill, copy of mortgage paper or rental agreement showing your name and the property address or Adult Residence Statement

  • Current custody papers, if applicable

  • Copy of latest grade card or interim report

  • Copy of ETR or IEP if applicable

  • Immunization Record

Please bring the above items with a completed Enrollment Packet to the enrollment appointment. Copies will be made and the originals returned to you during your appointment.
